Waiver Forms - Important Guidelines & Instructions

Welcome to the Waiver Forms page for Princess in Me Kids Spa! 

Before attending your spa party, all participants are required to complete a Consent and Waiver Form. This includes both the host and all party participants. This is an essential part of ensuring a safe and enjoyable experience for everyone involved. Please carefully review and sign the waiver forms below to ensure the process goes smoothly.  

We cannot begin the party until all participants have completed their waiver forms. If forms are not submitted or completed on the day of the event, there will be no extensions to the party time. To make the most of your spa experience, please ensure all forms are submitted before the scheduled party time.

It is the responsibility of the host or the parent/guardian of each child to ensure that all waiver forms are completed and submitted. If any participant does not have a completed waiver, they will not be able to participate in the spa services.
